Advocate Health in Wisconsin is seeking a Medical Social Worker at Aurora BayCare Medical Center in Green Bay. This part-time role requires 1–2 years in a clinical hospital setting post-licensure and a MSW from an accredited program.
IL/WI licensure eligibility or applicable certification is required. You will perform assessments, care planning, and discharge coordination, collaborating with a multidisciplinary team to ensure safe transitions and access to post-acute services.

Pay Range: $30.70 - $46.05

Licensure or Certifications: IL locations-LSW Eligible, LCSW preferred issued by the State of Illinois WI locations -Social Work Certification issued by the State of Wisconsin Education/Experience: Masters of Social Work services from an accredited school Years of Experience: 1-2 years of experience in a clinical, hospital setting post licensure preferred.
Schedule: This position is part of a job share, 40 hours over 2 weeks. 7:30am-4:00 pm. Weekend rotation (approximately 6 per year). On-Call rotation (approximately 15 days per year). Holiday rotation (no more than 1 holiday per year) Currently the position is Mon/Tues one week, then Wed/Thurs/Fri the other week.

This position requires travel, therefore, will be exposed to weather and road conditions. Operates all equipment necessary to perform the job. Exposed to a normal office environment. Weekend, holidays, and evening coverage per site requirement. Position requires sitting, standing, speaking, reading/writing, and walking throughout the workday.
