Qualifications

• Master's Degree in Social Work from an accredited institution.
• NY LMSW licensure or eligibility required.
• 6 months to 1 year of clinical experience in hospital, skilled nursing, or outpatient settings preferred.
• Strong communication, interpersonal and organizational skills.
• Proficiency with EHRs and documentation standards.
• Willingness to travel between MVHS worksites.

Responsibilities

• Conduct psychosocial assessments to understand patients' needs and develop care plans.
• Develop and implement individualized treatment plans with patients, families and the care team.
• Provide individual, group, and family therapy as appropriate.
• Assist in care coordination across hospital and outpatient settings to ensure continuity of care.
• Facilitate referrals to community resources and additional providers.
• Collaborate with physicians, nurses and other professionals for integrated care.
• Maintain accurate documentation and ensure compliance with regulations.
