Job summary

The Mohawk Valley Health System in Utica, NY seeks a Social Worker MSW to deliver comprehensive social work services across hospital, SNF, and outpatient settings. You will assess patients, develop and implement treatment plans, and provide therapy under supervision of licensed professionals.

Qualifications

Master's Degree in Social Work (MSW) from an accredited institution.
6 months to 1 year of clinical experience in healthcare settings.
Excellent communication, interpersonal, and organizational skills.
Proficiency with electronic health records (EHR) and documentation standards.
Ability to travel to MVHS worksites.

Responsibilities

Under a licensed professional, conduct psychosocial assessments to understand patients' needs.
Develop and implement individualized treatment plans with the healthcare team.
Provide individual, group, and family therapy as indicated.
Assist in care coordination across hospital, skilled nursing, and outpatient settings.
