Job summary

Carroll Hospital, part of LifeBridge Health, seeks a compassionate Hospice Registered Nurse to provide high-quality, patient-centered end-of-life care on our inpatient unit. The role may require cross-campus coverage to Carroll Hospital's hospice unit when census is low.

Qualifications

• Active RN license with Maryland Board of Nursing (MBON)
• AHA BLS Provider certification
• Reliable transportation to cover cross-campus shifts

Responsibilities

• Collect and document health status data from records, family input, and resources.
• Formulate, implement, and evaluate individualized care plans per agency policies.
• Prioritize and coordinate autonomous patient care.
• Provide emotional and clinical support to patients and families during end-of-life.
• Perform rounds with physicians and communicate care changes to the hospice team.
• Supervise Hospice Aides/HHAs and guide shift-by-shift planning.
• Participate in IDT meetings and maintain timely documentation.
• Support performance improvement with prompt documentation requests.
• Represent Carroll Hospice and Carroll Hospital in community liaison activities.
