Job summary

LifeBridge Health is seeking a compassionate Hospice Registered Nurse (RN) to provide patient-centered end-of-life care on our inpatient unit. The role may require cross-campus coverage to Carroll Hospital’s hospice inpatient unit during low census, offering autonomy and meaningful patient-family connections.

Responsibilities include data collection, care plan development, and collaboration with the interdisciplinary team to ensure high-quality, cost-effective care.

Qualifications

• Active RN license (MDMBON)
• AHA BLS Provider certification
• Reliable transportation to campus and assignments

Responsibilities

• Collect and document comprehensive health status data from patients, families, and records.
• Develop and evaluate individualized care plans in line with policies.
• Coordinate and prioritize care with autonomy while on shift.
• Provide emotional and clinical support to patients and families in the terminal phase.
• Perform rounds with physicians and communicate care changes to the hospice team.
