Job Duties and Responsibilities

• Assists patient with bathing, dressing, oral hygiene, hair treatments, nails and skin care.
• Assists patient with elimination using bedpan, urinal, commode or bathroom.
• Assists patient with eating and meal preparation, including special diets.
• Assists with positioning, transfers and ambulation, with or without assistive devices.
• Assists patient with prescribed range of motion and exercise taught by professional personnel.
• Assists patient with application of non‑prescription topical ointments.
• Takes and records temperature, pulse, respiration and blood pressure.
• Performs routine catheter care and removal when instructed by staff.
• Performs clean dressing changes and decubitus care.
• Assists with ostomy care—emptying pouch, changing appliance, preparing irrigation procedure.
• Charts tasks performed on designated records/reports.
• Observes tracheostomy and reports unusual findings to staff nurse.
• Communicates with patient families, agency staff to exchange verbal, nonverbal and written information.
• Observes and reports deviations from usual status or unusual occurrences to staff nurse promptly.
• Provides holistic care to hospice patients.
• Acts as preceptor for new employees during orientation.

Physical and Sensory Requirements

• Ability to sit for 2 hours per day, up to 1 hour consecutively.
• Must be able to stand for 10 hours per day, up to 8 hours consecutively.
• Must walk 6 hours per day, up to 10 minutes consecutively.
• Ability to use finger/hands frequently for handling.
• Ability to push objects up to 10 pounds frequently and occasionally up to 75 pounds.
• Ability to transport patients up to 250 pounds via wheelchair, stretcher or bed.
• Frequent stooping and bending required.
• Frequent reaching above shoulder level.
• Ability to feel objects with fingertips.
• Can hear normal conversation and high/low frequencies.
• Can see general, near, far, color and peripheral vision.

Education

High school diploma or GED required.

Successful completion of a training and competency evaluation program meeting Department of Health and Human Services standards is required.

Training and Experience

• Two years of Home Health Aide (HHA) experience under qualified supervision in a Medicare Certified program.
• Certified Nurse Aide, Personal Care Aide or Home Health Aide experience preferred.
• Ability to communicate in English.
