Job summary

Bristol Health Medical Group is seeking an experienced full-time Nurse Practitioner to join our inpatient Geriatric Medicine and Palliative Care program at Bristol Health. You will assess, diagnose, and manage older adult patients in collaboration with physicians, nurses, and interdisciplinary teams to ensure high-quality, patient-centered care.

Responsibilities include performing examinations, prescribing medications, coordinating care with specialists, and participating in quality improvement

Qualifications

• Graduate from an accredited Nurse Practitioner program.
• Board certification from a recognized body (AGPCNP/AGACNP/Family NP).
• Connecticut-licensed APRN authorized to practice independently.

Responsibilities

• Performs medical examinations, evaluations, assessments, diagnoses, consultations for inpatients.
• Prescribes pharmaceuticals and treatment regimens as appropriate.
• Collaborates with physicians, nurses and interdisciplinary teams.
• Directs and coordinates care between specialists and care components.
• Participates in health promotion, education and prevention programs.
