The Inpatient Coder assigns the principal diagnosis, comorbidities, complications, and chronic conditions for Hackensack Meridian Health (HMH) Inpatient cases across the network, using mandated coding systems in accordance with CMS and other regulations.

Position Details

This is a 100% remote position with flexible weekend work. The evening shift runs from 3-11:00 pm.

Responsibilities
  1. Assign ICD-10-CM and ICD-10-PCS codes, creating DRG assignments and abstracting pertinent information from patient records.
  2. Sequence diagnoses and procedures per guidelines to optimize DRG.
  3. Address edits related to nosology, PSI, HAC, and other quality indicators.
  4. Apply reason/tracking codes in 3M for DRG mismatches.
  5. Query physicians for unclear documentation or code assignments.
  6. Apply POA indicators on inpatient charts.
  7. Stay updated on coding guidelines, regulations, and new procedures.
  8. Follow ethical coding standards per AHIMA guidelines.
  9. Enter coded information into the designated system.
  10. Review case management notes for accurate discharge disposition.
  11. Identify and notify about missing operative/path reports.
  12. Participate in ongoing educational webinars and external reviews.
  13. Review audited cases and provide rebuttals if needed.
  14. Perform other duties as assigned.
Qualifications
Required
  • High School diploma or GED.
  • At least 3 years of experience in inpatient coding in a complex healthcare setting.
  • Expertise in medical terminology, anatomy, physiology, and pharmacology.
  • Proficiency in encoder software and computer skills.
  • Excellent communication skills.
  • Proficiency in Microsoft Office and Google Suite.
Preferred
  • Associate's degree or higher in HIM or related field.
Licenses and Certifications
  • Required: Certified Coding Specialist (CCS).
  • Preferred: RHIA, RHIT, or similar credentials with inpatient experience.
