Job summary

U.S. Department of Veterans Affairs is seeking an Inpatient Clinical Pharmacist to join the Sacramento Division.

The role focuses on drug therapy management, patient counseling, and interdisciplinary collaboration to ensure safe, effective pharmacotherapy for hospitalized veterans. The GS-13 level clinical pharmacist is expected to demonstrate expert knowledge of clinical pharmacy practice, manage medication therapy, and participate in rounds and committees.

Qualifications

• Minimum of 1 year inpatient experience preferred.
• Graduate of ACPE-accredited College or School of Pharmacy with a BS Pharmacy or Doctor of Pharmacy degree.
• Maintain full current license to practice pharmacy in a U.S. state or territory.

Responsibilities

• Evaluate inpatients for appropriateness of pharmacologic therapy.
• Develop and document pharmaceutical care plans and monitor therapy.
• Dispense medications accurately and ensure medication orders are appropriate and safe.
• Counsel patients to improve adherence and participate in discharge planning.
