Qualifications

• US citizenship is required.
• Full, current and unrestricted license to practice pharmacy in a US state, territory, or DC.
• English language proficiency in speaking and writing as required by VA policy.

Responsabilités

• Review, interpret, and verify medication orders for appropriateness.
• Process and fill medication orders and provide drug information to patients and healthcare teams.
• Perform medication reconciliation and monitor therapy effectiveness and safety.
