Job summary

Cleveland Clinic Mercy Hospital is seeking a Clinical Pharmacist – Inpatient to support Mercy's 24/7 pharmacy operations. Responsibilities include medication order verification, IV room support, renal dosing, and collaboration with the infusion center and oncology team.

The pharmacist will provide disease-state and medication education and complete histories to optimize patient care. The role entails a mix of day and evening shifts (about 60% days, 40% evenings), every third weekend rotation and

Qualifications

• PharmD or BS in Pharmacy from an accredited college.
• Current state licensure as a Registered Pharmacist or eligible for reciprocity within 90 days of hire.
• BLS certification through AHA or American Red Cross and completion of immunization administration course within 90 days (if required).
• ACLS and/or PALS certification through AHA or Red Cross, required within one year for certain roles.

Responsibilities

• Medication order verification and prescription fill verification.
• Renal dose monitoring, IV room support and compounding.
• Collaboration with infusion center and oncology team.
• Disease-state education, medication education and history intake to optimize therapy.

Education

Registered Pharmacist licensure
