Qualifications
Current Pharmacist’s license issued by the Oregon State Board of Pharmacy.
Training or experience in a specialized area may be required at recruitment.
Responsibilities
Identify, resolve, and document complex therapeutic problems.
Monitor therapeutic response and adverse effects to guide therapy.
Supervise technicians, students, interns, and residents.
Assist in medication reconciliation and discharge planning.
