Job summary

Qualifications

• Pharmacist license in Pennsylvania required.
• Pharmacist degree (PharmD or B.S./B.A. in Pharmacy) required.
• PGY1 Pharmacy Residency preferred or 2+ years as licensed inpatient pharmacist.

Responsibilities

• Prepare and dispense medications per hospital policies.
• Respond promptly to drug information requests from providers and patients.
• Ensure continuity of pharmaceutical care across inpatient and ambulatory settings.
• Participate in ADE assessment and reporting.
• Assist in medical emergencies and adhere to medication-use policies.
