Responsibilities

• Review, interpret, and verify medication orders for appropriateness.
• Process and fill medication orders.
• Interact with clinicians to optimize therapy and ensure safe care.
• Review patient medications, allergies, labs, and medical records for issues.
• Provide drug information and recommendations to staff; document interventions.
• Assist in formulary management and advise on substitutions.
