The inpatient diabetes educator provides timely and targeted education and support services to patients and support system. Bridges the gap between clinical care and self-mgmt. Delivering survival skills education, ensures a smooth transition from inpatient to outpatient. A subject matter expert in diabetes and technology, offers guidance to medical staff and participates in regular and ad-hoc education initiatives. Works collaboratively with interprofessional teams. Facilitates monthly diabetes support group. Credentialing as Diabetes Care and Education Specialist required within 1 year of hire. Must maintain 15 diabetes related CEU credits per year and active registration and licensure.

Education
Bachelors Degree: Nursing or Nutrition (Required)

Experience
3 Years experience as a Registered Nurse or Dietitian (Required)
General Experience working with a diverse patient population (Required)
3 Years experience in Diabetes self-management education and support experience (Preferred)
1 Year experience in an acute care setting (Preferred)

License/Certifications
LDN-LIC - PA Dietitian -Nutritionist (Required) or RN-LIC - PA Registered Nurse License (Required)
CDCES - Cert Diabetes Care and Ed Spec (Required)
