Sentara Princess Anne Hospital in Virginia Beach, VA, is hiring an Inpatient Case Manager/Social Worker (MSW) for a full-time, 40-hour/week role onsite. The position offers a sign-on bonus up to $15,000 for qualified candidates and requires an MSW with at least 1 year of acute care experience.

Schedule is 8:00 AM–4:30 PM EST, with rotating weekends. Responsibilities include assessing patient needs, coordinating with the healthcare team, and developing transition plans to support patient flow

Overview

Sentara Princess Anne Hospital is hiring an Inpatient Case Manager/ Social Worker - MSW! This position is eligible for up to a $15,000 sign-on bonus for qualified candidates.

Status

Full-Time, permanent position (40 hours)

Standard working hours

8:00 AM - 4:30 PM EST, Monday-Friday, with rotating weekends (Saturday or Sunday). A weekday off is provided when working a weekend day so hours do not exceed 40 per week.

Location

This is an onsite position at Sentara Princess Anne Hospital, 2025 Glenn Mitchell Drive, Virginia Beach, VA 23456.

Job responsibilities

• Responsible for the provision of medical social work services to patients receiving care in the hospital setting.
• Assesses the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient.
• Collaborates with healthcare team members for the patient and community organizations to identify needs and resources in the hospital and community.
• In consultation with the healthcare team develops effective transition plans.
• Collaborates with the RN Case Manager to ensure a smooth flow of patients through their hospital stay.
• Functions in one of the following practice settings: Hospitals Only

Education

MLD - Master's Level Degree in Social Work / MSW REQUIRED

Certification/Licensure

• Basic Life Support (BLS) - Certification within 90 days of hire is REQUIRED
• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is REQUIRED within one year of eligibility.

Experience

1 year of acute care and/or healthcare experience requirement is defined below: Educational internship experience in an acute care facility can be considered OR Healthcare experience refers to the care delivered in one of the following settings: long-term care, home health, hospice, Skilled Nursing Facility (SNF)

Compensation

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $72,737.60 - $110,323.20 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
