Inpatient Case Manager RN, BSW or MSW (Full Time/Days)

LOCATION: Lancaster PA

HOURS: Full-time, day shift between 7:30 am-4:30 pm, with 2-3 weekends per year.

SIGN ON BONUS: $5,000 with a 2-year commitment. External applicants only.

Position Summary
  • Acts as an advocate for patients, assisting them to reach optimal health in environments supporting their needs. The Case Manager functions within the full scope of practice, extending responsibilities beyond the episode of care to coordinate across the continuum, applying assessment, planning, implementation, monitoring, evaluation, and coordination of care in collaboration with health team members and community resources to ensure safe, quality, and cost-effective care.
  • Conducts assessments of patient and family needs, coordinating input from all health professionals to formulate a documented plan ensuring continuity of care for high-risk patients.
  • Delegates care coordination tasks based on the situation, supporting assistive personnel, and holds them accountable for delegated tasks.
  • Identifies appropriate providers, facilities, and external healthcare organizations, communicating with interdisciplinary teams to develop and maintain positive relationships with patients, families, and providers.
  • Coordinates care across inpatient, outpatient, and community settings to ensure appropriate utilization of resources.
  • Oversees and guides development of health partnerships to improve health outcomes.
  • Participates in data collection, health outcomes reporting, clinical audits, and program evaluations.
  • Identifies patients at risk for poor outcomes or experiencing poor coordination who would benefit from intensive follow-up.
  • Provides chronic disease management and self-management education and support.
  • Assists in developing practice parameters, identifies professional development needs, and participates in committees and performance improvement activities.
  • Engages in educational activities with medical students and residents and participates in on-call duties as required.
Minimum Qualifications
  • RN, MS in health-related field, or Social Worker with current licensure and relevant experience.
  • For RN: Current Pennsylvania licensure, BSN or related degree, or 10+ years of acute/subacute care or case management experience.
  • For Social Worker: MSW or BSW with MS in Human Services, or BSW with 10+ years relevant experience.
Additional Requirements
  • Three years of acute care, social work, or case management experience.
  • Excellent communication and customer service skills.
  • Proven leadership and ability to work independently and in teams.
  • Highly organized, detail-oriented, and capable of multitasking.
  • Experience with motivational interviewing, teach-back, and self-management support.
  • One year of case coordination and project implementation experience.
Preferred Qualifications
  • Five years of case management experience.
  • Case Management certification (ACM or CCM) within three years.
  • Knowledge of utilization review and managed care.
