Qualifications

• Bachelor’s Degree in Nursing preferred.
• RN licensure in VA or NC or Compact/Multi-state is REQUIRED within terms.
• BLS certification within 90 days of hire.
• Case Management certification (ACM/CCM/CCCTM/CMAC/CMGT-BC) within one year of eligibility.

Responsibilities

• Manage comprehensive care across the hospital with focus on patient transitions.
• Promote appropriate utilization and monitor health services.
• Collaborate with healthcare team and patient/caregiver to manage care and transitions.
• Develop and implement care plans addressing physical, mental, and psychosocial needs of inpatients.

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN) - Part Time Weekend Dayshift! This position is eligible for up to a $10,000 sign-on bonus for qualified candidates.

Status and Hours

Status: Part-Time, permanent position (32 hours). Standard working hours: 8:00 AM – 6:30 PM EST OR (8:00 AM – 6:30 PM EST), Friday, Saturday, and Sunday OR Saturday, Sunday, and Monday. One 12-hour shift and two 10-hour shifts, including a 30-minute lunch break. Eligible for weekend differential. The position requires training Monday through Friday from 8:00 AM – 4:30 PM EST for the first several weeks before transitioning to the part-time weekend day shift.

600 Gresham Drive, Norfolk, VA 23507. This is an onsite position at Sentara Norfolk General Hospital.

Job Responsibilities

The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only

Education

Bachelor's Degree in Nursing preferred

Certification/Licensure

Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED BLS within 90 days of hire. Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.

Experience

• 1 year of nursing experience
• 1 year acute care case management preferred

Compensation

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits.

The base pay range for full-time employment is $89,544.00 - $136,468.80 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities.

Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
