Sentara Health is seeking an Inpatient Case Manager RN at Sentara Norfolk General Hospital in Norfolk, VA. This full-time weekend day shift role offers 3x12 or 4x10 schedules with stable hours and a $20,000 sign-on bonus for qualified candidates.

You will coordinate care across the hospital, develop care plans, and collaborate with the care team to ensure appropriate transitions and LOS. Requires RN license in VA/NC or multi-state, BLS, and a ACM/CCM/CCCTM/CMAC/CMGT-BC certification within one

Qualifications

• Bachelor's Degree in Nursing preferred.
• Registered Nurse (RN) licensed in VA or NC or Compact/Multi-state.
• BLS certification within 90 days of hire.
• Case Management certifications (ACM, CCM, CCCTM, CMAC, or CMGT-BC) within one year of eligibility.
• 1 year of nursing experience; 1 year acute care case management preferred.

Responsibilities

• Provide and facilitate comprehensive care management and quality outcomes for inpatients.
• Support hospital patients needing case management and coordinate care transitions.
• Develop and implement comprehensive care plans addressing physical, mental, and psychosocial needs.

Inpatient Case Manager/ Registered Nurse (RN) – Full Time Weekend Dayshift

This position is eligible for up to a $20,000 sign-on bonus for qualified candidates. Status: Full-Time, permanent position (36 hours OR 40 hours) Standard working hours: 3x12 weekend schedule/ 36 hours: 8:00 AM to 8:30 PM EST, Friday, Saturday, and Sunday OR Saturday, Sunday, and Monday, including a 30-minute lunch break 4x10 schedule/ 40 hours: 8:00 AM to 6:30 PM EST, Friday through Monday, including a 30-minute lunch break Eligible for weekend shift differential The position requires training Monday through Friday from 8:00 AM – 4:30 PM for the first several weeks before transitioning to the full-time weekend day shift. Location: This is an onsite position at Sentara Norfolk General Hospital, 600 Gresham Drive, Norfolk, VA 23507.

Job Responsibilities

The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only

Education

Bachelor's Degree in Nursing preferred

Certification / Licensure

Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED BLS within 90 days of hire. Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.

Experience

1 year of nursing experience 1 year acute care case management preferred

Compensation

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $89,544.00 - $136,468.80 annually. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
