Sentara Careplex Hospital in Hampton, VA, is seeking an experienced Inpatient Case Manager MSW to focus on complex cases!

Inpatient Case Manager MSW - Complex Cases

Sentara Careplex Hospital - Hampton, VA

~Full Time & Day Shift~

40 hours/week

Sign On Bonus up to $15,000 available for qualified candidates!

Hours

• Monday - Friday: 8:30am - 5pm, with a weekend rotation approximately every 4 weeks, plus holiday and on-call commitments.

Overview

Responsible for providing medical social work services to hospital patients. Assesses social, cultural, environmental, and financial situations, as well as disposition needs. Collaborates with healthcare team members and community organizations to identify needs and resources. Develops transition plans in consultation with the healthcare team and ensures smooth patient flow during hospital stay in collaboration with the RN Case Manager.

Education

• Master's Degree in Social Work

Certification/Licensure

• Certification in Case Management (CCM, CMAC, or ACM-SW) required within one year of eligibility.

• BLS certification required within 90 days of hire.

Experience

• At least 1 year of related acute care or healthcare experience, including educational internships in acute care settings, is required.

• Healthcare experience should be in hospital or acute care settings.
