Overview

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager MSW to work full-time. Generous sign‑on bonus for qualifying candidates.

Location: Norfolk, VA

Work Shift: Rotating

Hours: Full time, 40 hours per week

Responsibilities

Responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, psychological, cultural, environmental, and financial situations, as well as disposition needs for each referred patient. Collaborates with patients, families, healthcare team members, and community organizations, and uses strong advocacy skills to identify needs and resources in the hospital and community. Develops and evaluates effective transition plans taking into consideration the client diagnosis, prognosis, course of treatment, past and present services, short‑term and long‑term goals, provider options, and/or available health‑care benefits as appropriate to the continuum of care. Functions in a hospital setting.

Requirements

• Master’s Degree in Social Work (MSW)
• 1 year of acute care and/or healthcare experience (education internship experience in an acute care facility considered)
• Certification in Case Management (CCM, CMAC, or ACM‑SW) required within one year of eligibility
• BLS required within 90 days of hire
