Fuller Hospital is seeking a Clinical Social Worker to join our inpatient and partial hospital teams in Massachusetts. You will provide a full range of social work services to diverse patients and collaborate with treatment teams to support care planning.

The role emphasizes discharge planning and aftercare coordination, acting as a primary liaison with external providers to ensure smooth transitions post-discharge.

Qualifications

• Master’s Degree in Social Work, counseling, or other human services related field.
• Licensed or eligible for LCSW, LICSW, LMFT, or LHMC.
• Prior hospital social work experience preferred.

• Provide a comprehensive range of social work services to a variety of patients in an inpatient/partial hospital setting.
• Act as the primary contact with external providers and actively participate in each assigned patient’s treatment plan.
• Be the primary individual responsible for patient discharge and aftercare planning.

Clinical Social Worker Opportunity

Fuller Hospital is a 109-bed licensed, private psychiatric facility located in South Attleboro, Massachusetts providing inpatient and outpatient behavioral health services to residents of Massachusetts and Rhode Island. Inpatient units are designed to treat adults with general psychiatric or intellectual disabilities, as well as adolescents with general psychiatric issues. Patients with co-occurring psychiatric and substance abuse disorders can be treated on most units. Fuller Hospital also provides a less intensive Partial Hospitalization Program (PHP) to patients with psychiatric and co-occurring psychiatric and substance abuse disorders.

Responsibilities

• Provide a comprehensive range of social work services to a variety of patients in an inpatient/partial hospital setting
• Act as the primary contact with external providers and actively participate in each assigned patient’s treatment plan
• Be the primary individual responsible for patient discharge and aftercare planning
