Job summary

• BSN or actively enrolled in BSN with completion 2 years from hire date
• RN licensure in Illinois
• CPR certification
• Minimum of 2 years of recent acute care nursing experience preferred

Responsibilities

• Coordinates care from admission to discharge for assigned patients
• Collaborates with interdisciplinary care team and physicians to develop and implement plan of care
• Identifies and removes barriers to achieve patient outcome goals and coordinates appropriate discharge plan
