Inpatient Care Manager (RN): Per Diem Days, 8AM-4PM, Morristown Medical Center

Atlantic Health System is seeking a dynamic Registered Nurse to join the energetic and innovative team of healthcare professionals as an Inpatient Care Manager on the Care Management Team at Morristown Medical Center. This is a per diem position where the candidate will provide services during the day, Monday through Friday, with occasional weekends. The ideal candidate will be responsible for providing care and services to selected patient populations across the continuum of illness, ensuring effective utilization of healthcare resources, managing the interdisciplinary team to achieve optimal clinical and resource outcomes, and working with the Supervisor/Manager of Case Management to assess, plan, implement, coordinate, monitor, and evaluate services and outcomes to maximize patient health.

Principal Accountabilities
  1. Manage, coordinate, and facilitate patient care through assessments and multidisciplinary collaboration.
  2. Conduct daily chart reviews, utilization reviews, discharge planning, and comprehensive clinical documentation in the electronic medical record.
  3. Participate in interdisciplinary patient care rounds, identify and resolve care quality issues, barriers, and discharge delays.
  4. Develop and coordinate discharge plans and care strategies to ensure safe, timely, and effective discharges.
  5. Collaborate with the healthcare team to manage and assist with patient care plans.
  6. Perform variance tracking and outcome analysis to evaluate clinical pathway effectiveness.
  7. Represent the department and hospital outreach initiatives.
  8. Perform other related duties as assigned.
Qualifications
  • Bachelor's degree in Nursing or related field.
  • Valid New Jersey Nursing License.
Preferred Qualifications
  • BSN preferred.
  • Minimum of 1 year of acute care experience preferred.
