Horizon Health in Abbeville, LA seeks a Clinical Assessment Coordinator (PRN) for a 16-bed inpatient behavioral health unit at Abbeville General Hospital. The role conducts referral development, pre-admission assessments, and complex clinical evaluations under supervision, with a strong emphasis on multidisciplinary collaboration.

The candidate should have a Bachelor’s or Master’s in a relevant field, and hold RN/LPC/LMSW credentials with 3–5 years in intake and crisis prevention experience.

Qualifications

• Bachelor’s degree in nursing or Master’s degree in social work/counseling is required.
• RN, LPC, or LMSW license required.
• 3–5 years of intake experience required.

Responsabilités

• Diagnose, implement, coordinate and evaluate clinical admission, referral and/or discharge processes.
• Conduct on-site and off-site patient assessments with appropriate test administration and recommendations.
• Develop treatment plans in coordination with the Treatment team and communicate with physicians and other agencies.
• Enter data into HorizonPlus system and generate reports on referral, admission, and discharge trends.
• Perform other duties as assigned.
