Job description

LOCATION: Remote - must live in or near McDowell or Yancy County, North Carolina. The person in this position is required to maintain residency in North Carolina or within 40 miles of the NC border. This position requires travel.

GENERAL STATEMENT OF JOB

The Innovations Care Manager (Innovations CM) is responsible for providing proactive intervention and care coordination to eligible Vaya Health members to ensure they receive appropriate assessments and services. The Innovations CM collaborates with members and care teams to address care gaps, manage assessments, develop multidisciplinary care plans, and coordinate services across various health and social systems. This role involves supporting transitions, tracking discharged individuals, and working in diverse community settings, including members' homes. The Innovations CM also engages with staff, family, caregivers, providers, and community stakeholders. Essential job functions include:

  • Utilizing Vaya's Care Management software and health record systems
  • Engaging in outreach and member engagement activities
  • Ensuring HIPAA compliance and proper information release practices
  • Conducting comprehensive Health Risk Assessments (HRA)
  • Maintaining medication lists and ensuring continuity of care
  • Participating in care team meetings and care planning
  • Supporting transitional care and diversion from institutional placements

Residency Requirement: Must reside in North Carolina or within 40 miles of the border to effectively deliver in-person services.

ESSENTIAL JOB FUNCTIONS

Assessment, Care Planning, and Care Team Collaboration:

  • Identify, assess, and develop person-centered care plans addressing mental health, substance use, medical, and social needs
  • Link members with appropriate services and supports
  • Conduct HRAs and screenings, including medication reviews and care plan development
  • Involve members and care team in planning and review processes

Support Monitoring, Documentation, and Fiscal Accountability:

  • Participate in multidisciplinary team meetings and case staffing
  • Ensure timely and accurate documentation in systems
  • Monitor service delivery and compliance with standards

Other duties as assigned

KNOWLEDGE, SKILLS, & ABILITIES

  • Effective communication and interpersonal skills
  • Ability to drive and work in diverse community settings
  • Strong organizational skills and attention to detail
  • Proficiency in Microsoft Office and care management systems
  • Knowledge of mental health, substance use, and IDD/TBI service systems
  • Experience with NC Medicaid programs and related regulations is preferred

EDUCATION & EXPERIENCE

Bachelor's degree in a relevant field required. Experience requirements vary based on education:

  • 2 years with I/DD or TBI populations if holding a Bachelor's in Human Services
  • Additional experience required if holding degrees in other fields or licensure as RN

*Must meet North Carolina Qualified Professional criteria as per 10A NCAC 27G .0104

Licensure/Certification: RN license in North Carolina if holding a nursing degree.

PHYSICAL REQUIREMENTS

  • Visual acuity, mobility, and ability to perform physical activities as needed
  • Sedentary work with lifting up to 10 pounds, extended sitting
  • Ability to drive and work in rural areas

SALARY: Commensurate with qualifications and experience. Non-exempt position eligible for overtime.
