Remote - must live in or near Henderson County, North Carolina. Incumbent in this role must live in North Carolina or within 40 miles of the NC border. This position requires travel.
The Innovations Care Manager (Innovations CM) is responsible for providing proactive intervention and care coordination to eligible Vaya Health members to ensure appropriate assessment and services. The Innovations CM collaborates with members and care teams to address care gaps across various health domains, including mental health, substance use, intellectual/developmental disabilities, traumatic brain injury, physical health, pharmacy, LTSS, and unmet health-related needs. This role involves supporting transitions from facilities, tracking discharge follow-ups, and working in members' home communities. The position requires travel and is mobile, working across multiple locations. The Innovations CM works with staff, members, families, providers, and community stakeholders. This position requires NC residency or living within 40 miles of the NC border to effectively deliver in-person contacts.
Includes assessment, care planning, interdisciplinary team collaboration, support monitoring, documentation, fiscal accountability, and other duties as assigned. Key responsibilities involve conducting assessments, developing person-centered care plans, supporting members' access to services, participating in multidisciplinary meetings, monitoring service delivery, ensuring documentation accuracy, and supporting crisis intervention and transition planning.
Bachelor's degree in a related field; specific experience with I/DD, TBI, LTSS, and care management is required, with variations depending on the degree held. Must meet NC Qualified Professional criteria.
If holding a Nursing degree, must be licensed by the NC Board of Nursing.
Residency in NC or within 40 miles of the border is required. Salary depends on qualifications; position is non-exempt with overtime eligibility.
