Job summary

Bernalillo County is hiring a Social Services Coordinator (MDC) to manage inmate casework, assess needs, and connect individuals with reentry resources. The role requires collaboration with MDC staff and external providers, ensuring accurate documentation and compliance with policies.

Responsibilities include interviewing inmates, maintaining files, and coordinating benefits enrollment, with a flexible schedule and a focus on patient-centered reentry planning.

Qualifications

• Bachelor’s degree in related field.
• 3 years of work experience in criminal justice, social work, counseling, case management or related field; or 7 years in related areas.
• Knowledge of correctional program operations and the court system.
• Knowledge of community social services and reentry benefits.
• Knowledge of counseling and case management techniques.
• Knowledge of determining security levels and inmate rights.
• Ability to use computers to generate reports and maintain records.
• Ability to interview and evaluate inmates for eligibility; interpret court documents.
• Clear written and spoken English communication.
• Flexible schedule including evenings and weekends.

Responsibilities

• Conducts case management services for assigned inmates; assesses needs including drug/alcohol, DV, behavioral health, and reentry resources.
• Acts as key contact for enrollment for benefits/eligibility programs.
• Performs initial interviews with inmates; reviews history; prepares and updates inmate files with proper discharge planning.
• Maintains documentation and reporting of case management processes.
• Creates and maintains partnerships to facilitate social services initiatives and outcomes.
• Serves as liaison with community benefit providers and MDC.
• Monitors program compliance with laws, rules and procedures.
• When at CCP, acts as liaison and facilitates classification to CCP by court order; ensures timely follow-up and reporting.
• Performs other related duties as required.
