Memorial Health in Illinois is seeking a Registered Nurse-Admission Liaison for Infusion to assess patient needs after referrals and coordinate between hospital staff and infusion/home health teams. You will explain program services to patients and families and serve as a resource for affiliates and nursing staff on infusion, Medicare coverage, and admission requirements.

The role requires an ASN, IL RN licensure, and BLS; 3+ years nursing experience is required with infusion/home health

• Explain MH program services to patient and family.
• Coordinate with MH intake, BJC pharmacy, and Home Health intake as needed.
• Educate physician and family about infusion, Medicare coverage, and admission limits.
• Provide bedside teaching to patients and caregivers.
