Job summary

Catholic Health Home Care is seeking an experienced Infusion RN to deliver and coordinate home health infusion therapy, developing care plans with physicians, patients, and caregivers. You will educate patients on treatment, medications, and health management, and ensure interdisciplinary collaboration to provide holistic, patient-centered care in the home setting.

The ideal candidate will hold New York State RN license, have at least one year of hands-on RN experience, BLS, and a valid NY

Qualifications

• New York State RN license.
• Minimum of one year of recent hands on experience as a Registered Nurse.
• Basic Life Support Certification.
• Valid New York State Driver’s License and reliable transportation.

Responsibilities

• Deliver and coordinate home health infusion therapy.
• Develop and update a care plan in collaboration with physicians, the patient, and caregivers.
• Educate patients and caregivers on treatment, medication, and health management.
• Ensure interdisciplinary collaboration for holistic care delivery.
