Job summary

Enliven is seeking an experienced RN to service home infusion and Infusion Center patients in Westfield, IN and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care with little supervision, educating patients and families on prevention and healthy habits.

Responsibilities include EMR documentation, admission and case management, infusion administration, follow-up, and discharge planning.

Qualifications

• RN with Associate or BSN required.
• CPR/BLS required; ACLS/PALS preferred.
• Strong clinical skills in Med/Surg, ICU, ER and infusion.
• Ability to educate patients and families on prevention and healthy habits.

Responsibilities

• Provide admission, case management and follow-up visits for home infusion and infusion center patients.
• Administer ongoing care and infusions; follow physician orders and clinical manager guidance.
• Document patient care in EMR and discharge summaries.
• Coordinate plan with physicians, pharmacies and care team.
• Discharge patients after physician/manager approval and finalize documentation.
