Job summary

Enliven Home Infusion Specialists Colorado is seeking an experienced RN to service home infusion and infusion center patients in Windsor and surrounding locations. The RN will educate patients and families on prevention and healthy habits, provide high-quality nursing care with minimal supervision, and handle emergent situations with compassion and professionalism.

Qualifications

• RN with Associate or Bachelor’s in Nursing.
• Minimum five years of nursing experience preferred in an Acute Care setting.
• Current CPR/BLS required; ACLS/PALS preferred.
• Home health/infusion experience preferred.

Responsibilities

• Complete daily electronic forms for each assigned patient within a secure EMR system.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions and follow up as directed by the Physician and Clinical Manager.
• Collaborate with physicians and pharmacies to develop the initial treatment plan based on orders and assessment.
• Provide hands-on care, management, and teaching of the patient under a physician-based plan.
• Report patient care, conditions, and progress to the physician, pharmacy, and Clinical Manager.
• Implement patient care plan with patient and family to assist in resolving needs.
• Discharge patients after consultation with physician and Clinical Manager, complete documentation.
• Prepare medical documentation including case conferences, patient contacts, medication changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update patient records, and confer with other healthcare disciplines to provide optimal care.
