Job summary

Venture Forthe in New York is seeking a dedicated Home Infusion RN to provide specialized infusion therapy in patients' homes. You will initiate and monitor IV therapies, tailor infusion plans, and educate patients and caregivers about treatments and line care.

Travel is required across the service area, with emphasis on autonomy, collaboration with care teams, and ongoing competency development. The role supports a mission-driven team focused on compassionate, high-quality patient care.

Qualifications

• Current New York State RN license.
• Graduate of an accredited nursing program.
• At least 1 year of acute care nursing experience preferred.
• Infusion therapy experience strongly preferred (IV therapy, PICC lines, central line care, biologics, TPN, IV antibiotics, hydration).
• Home care experience preferred but not required.
• Strong communication, organizational, and critical thinking skills.
• Ability to work independently in varied home environments.

Responsibilities

• Provide specialized home infusion therapy services, including IV initiation, administration, monitoring, dressing changes, blood draws, and pump programming.
• Assess patients and develop individualized infusion plans of care.
• Educate patients and caregivers on infusion therapy, medications, line care, and treatment expectations.
• Collaborate with providers, pharmacies, and interdisciplinary care teams to ensure safe and effective care.
• Monitor patients for infusion reactions, side effects, and clinical changes.
• Maintain accurate and timely clinical documentation.
• Support quality improvement initiatives and ongoing competency programs.
• Assist with coordination care between physicians and specialty pharmacies
