Job summary

Venture Forthe is seeking a Home Infusion RN to provide specialized infusion therapy in patients' homes across the service area. You will initiate IV therapy, monitor patients, and document care plans.

The role emphasizes independence, ongoing education, and collaboration with physicians, pharmacists, and care teams to ensure safe, effective treatment. Travel is required within the service area, with a focus on quality patient outcomes.

Qualifications

• Current New York State Registered Nurse (RN) license.
• Graduate of an accredited nursing program.
• At least 1 year of acute care nursing experience preferred.
• Infusion therapy experience strongly preferred, including IV therapy, PICC lines, central line care, biologics, TPN, IV antibiotics, and hydration therapy.
• Home care experience preferred but not required.
• Strong communication, organizational, and critical thinking skills.
• Ability to work independently in varied home environments.

Responsibilities

• Provide specialized home infusion therapy services, including IV initiation, administration, monitoring, dressing changes, blood draws, and pump programming.
• Assess patients and develop individualized infusion plans of care.
• Educate patients and caregivers on infusion therapy, medications, line care, and treatment expectations.
• Collaborate with providers, pharmacies, and interdisciplinary care teams to ensure safe and effective care.
• Monitor patients for infusion reactions, side effects, and clinical changes.
• Maintain accurate and timely clinical documentation.
• Support quality improvement initiatives and ongoing competency programs.
• Assist with coordination care between physicians and specialty pharmacies
