Job summary

Advocate Health in Charlotte, NC is seeking a pharmacist to oversee pharmacotherapy and patient counseling in an oncology infusion setting. You will review orders, design regimens, and collaborate with care teams to ensure safe medication-use processes.

Candidate must hold a PharmD or NABP credential with SC licensure within 6 months of hire; immunization certification may be required. Oncology infusion experience is preferred and the role includes teaching responsibilities and quality

Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity.
• South Carolina license within 6 months of hire.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.
• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school or NABP FPGEC.
• Oncology, hospital or infusion experience preferred.

Responsibilities

• Oversees pharmaceutical care, including evaluating literature and designing medication therapy regimens.
• Provides counseling to patients and caregivers on therapy, adverse effects, compliance, and administration.
• Designs and monitors patient-specific pharmacotherapeutic regimens and ensures proper documentation.
• Interprets medication orders accurately and participates in medical emergencies when applicable.
• Supports medication safety and quality improvement activities, including reporting events.
• Collaborates with healthcare team on formulary adherence and medication-use processes.
