Job summary

Option Care Health, Inc. seeks an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite under a physician’s plan. The role operates under structured supervision by the designated supervisor.

The nurse will perform treatments, educate patients and families, and ensure continuity of care with the clinical team while complying with HIPAA and regulatory standards.

Qualifications

• Active RN license in the state of practice.
• Current CPR certification required.
• BSN degree preferred.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating progress and educating patients and families.
• Assesses patient needs and status at each visit, re-evaluates and updates physician orders as needed.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Teaches patients and families to achieve independence with prescribed therapy and care needs.
• Maintains real-time clinical documentation during visits and submits for billing per policy.
• Coordinates discharge planning and prepares summaries with instructions for care post-visit.
