Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification.

Responsibilities

• Initiates and implements intermediate nursing plans of care and evaluates progress.
• Provides direct patient care, administers medications, and educates patients/families.
• Assesses patient needs at each visit and updates plan per physician orders.
• Documents care in real time and communicates with the clinical team.
• Ensures compliance with policies, regulations, and billing timelines.
• Oversees paraprofessionals in home settings and may supervise in absence of manager.
• Participates in team conferences, precepting, and on-call responsibilities.
• Performs other related duties as assigned.
