Job summary

Soleo Health Inc is seeking a Per Diem Infusion Nurse to support infusion patients in Longmont, CO and surrounding areas. You will provide direct home-based infusion care, coordinate with the care team, educate patients, and ensure safety in the home setting. This role requires RN licensure in Colorado, with BSN preferred, and 3+ years infusion experience. Mileage reimbursement offered.

Qualifications

• Current RN license by the state’s Board of Nursing (Colorado).
• RN degree; BSN from an accredited institution preferred.
• Advanced certification in specialty areas (e.g., CRNI, ONS, IgRN) preferred but not required.
• Administrative oversight: monitor workflows, ensure policy compliance, manage resources.
• Emergency availability: respond promptly to emergencies, including non-standard hours.
• Minimum 3 years nursing experience with infusion therapy background preferred.
• ICU and/or Cardiac Care experience preferred.
• MS Outlook and Office proficiency; knowledge of AlayaCare, Insight, CPR+, Concur, Paylocity helpful.
• Valid driver’s license with no significant restrictions.

Responsibilities

• Provide direct patient care for assigned patients, including communication with care team members.
• Educate patients and care partners to safely administer home infusion therapy.
• Assess home environments and intervene to ensure safety and suitability for prescribed care.
• Develop and evaluate plans of care with the team; monitor response to therapy.
• Manage scheduling and coordinate patient care via approved platforms.
• Participate in nurse on-call rotation to ensure continuous support.
• Document all encounters and care activities per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and standards.

The Position:

The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities include:

• Provide direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.
• Evaluate and educate patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.
• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
• Develop in conjunction with other team members, the plan of care and evaluate response to therapy with each encounter.
• Manage schedule via company approved platform per company policy. Communicate any scheduling changes promptly to appropriate team members.
• Coordinate patient care via approved company platform.
• Participate in the nurse on-call rotation to ensure continuous nursing support services to our customers.
• Document all patient encounters, communications, and care activities at time of service as appropriate per Soleo policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
• Work in conjunction with appropriate departments to ensure all required patient documents (i.e. consents, POTs) are completed and returned to the branch office within an acceptable timeframe.
• Perform other duties as assigned.

Schedule:

• Ability to work Monday-Friday 8:30am-5pm- variable schedule to include: days, afternoons, evenings, and weekends as needed- Per Diem- 10-20 hours per week
• Must have a minimum of 2-3 days availability to see patients during the week
• Must be knowledgeable and have used the following IV access devices to include: IV lines, accessing ports, and administration of Infusion medications (to include PIV skills, central lines, PICCS, troubleshooting infusion pumps)
• Must be able to drive up to 1 hour to see patients in their homes (we offer mileage reimbursement and hourly rate of pay- even when driving!)
• Must be comfortable administering infusions to patients in their homes
• Must be licensed as an RN in Colorado - in good standing

Requirements

• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required
• Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies, and managing resources efficiently to meet organizational objectives
• Emergency Availability: Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations. Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency
• Minimum 3 years nursing experience with background in infusion therapy preferred
• Experience in ICU and/or Cardiac Care preferred
• Proficient with Microsoft Outlook and Microsoft Office Suite.Additional computer knowledge of the following programs: AlayaCare, Insight, CPR+, Concur, and Paylocity helpful
• Valid state issued driver’s license with no significant restrictions on that license
