Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Huntington, WV and surrounding areas. You will provide direct home-based care, educate patients, assess environments, and coordinate with the care team to ensure safe, effective infusion therapy.

The role requires an active RN license (multistate preferred), 3+ years of nursing experience with infusion background, and flexibility for variable hours including evenings and weekends.

Qualifications

• Current RN license by state boards, in good standing (multistate allowed).
• BSN preferred; RN degree from an accredited program.
• 3+ years nursing experience, infusion therapy preferred.
• ICU/Cardiac care experience preferred.
• Advanced certifications (CRNI/ONS/IgRN) preferred but not required.

Responsibilities

• Provide direct care for infusion patients in home/alternate sites.
• Educate patients and care partners on infusion therapy and safety.
• Assess home environments for safety and appropriateness of care.
• Develop and evaluate plan of care with the care team.
• Coordinate care and maintain documentation per policy.
• Participate in on-call rotation to support 24/7 nursing coverage.
• Maintain knowledge of Soleo therapies, protocols, and standards.

The Position:

The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities include:

• Provide direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.
• Evaluate and educate patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.
• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
• Develop in conjunction with other team members, the plan of care and evaluate response to therapy with each encounter.
• Manage schedule via company approved platform per company policy. Communicate any scheduling changes promptly to appropriate team members.
• Coordinate patient care via approved company platform.
• Participate in the nurse on-call rotation to ensure continuous nursing support services to our customers.
• Document all patient encounters, communications, and care activities at time of service as appropriate per Soleo policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
• Work in conjunction with appropriate departments to ensure all required patient documents (i.e. consents, POTs) are completed and returned to the branch office within an acceptable timeframe.

Schedule:

• Ability to work Monday-Friday 8:30am-5pm- variable schedule to include: days, afternoons, evenings, weekends as needed- Per Diem 3-10 hours per week
• Ability to drive your own properly registered/insured vehicle up to 75 miles one direction as needed (we offer mileage reimbursmenet and hourly rate of pay- even when driving!!)
• Must be knowledgeable and have used the following IV access devices to include: IV lines, accessing ports, and administration of infusion medications
• Must be comfortable working in patients homes around animals/cigarette smoke
• Must have a multistate RN license- in goodstanding

Requirements

• Current RN license by the applicable state’s Board of Nursing, in good standing- multistate RN license
• RN degree, bachelor’s degree in nursing (BSN) from an accredited institution preferred
• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
• Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies and managing resources efficiently to meet organizational objectives.
• Emergency Availability: Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations. Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency.
• Minimum 3 years nursing experience with background in infusion therapy preferred.
• Experience in ICU and/or Cardiac Care preferred.
• Proficient with Microsoft Outlook and Microsoft Office Suite - beneficial to have knowledge of: CPR+, AlayaCare, Insight, Concur, and Paylocity
• Valid state issued driver’s license with no significant restrictions on that license
