Job summary

Indiana University Health System in Bedford, IN, is seeking a part-time Infusion Clinical Pharmacist. This hybrid role schedules Tue, Wed, Thu 8am–4pm, with flexibility.

The pharmacist will deliver value-based medication care, collaborate with care teams, and support medication therapy outcomes to improve patient quality of life. The position requires PharmD or BS in Pharmacy, ASHP residency, Indiana licensure or eligibility, and 0–3 years of experience, with potential travel and weekend

Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy completed prior to 2006.
• ASHP-accredited PGY1 or PGY2 residency may be required.
• 0-3 years of relevant experience.
• Current Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist.
• Board certification or other specialized training may be required.
• Basic Life Support certification through the American Heart Association may be required.
• May require travel to IU Health facilities.
• May require weekend rotations.

Responsibilities

• Provide comprehensive medication-related care to improve patient quality of life.
• Collaborate with other healthcare professionals for medication therapy outcomes.
• Ensure safety and effectiveness of the medication-use process and dispense medications as ordered.
• Provide medication information support to medical and nursing staff and maintain patient confidentiality.

Description

This is a Part-time Infusion Clinical Pharmacist position located in Bedford, IN. This is a hybrid position with a schedule of Tuesday, Wednesday, Thursday 8am-4pm, but can be flexible.

Provides comprehensive, value-based medication-related care to achieve definite outcomes that improve a patient's quality of life. Acts in collaboration with other healthcare professionals in the shared responsibility for patient medication therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, maintaining oversight over pharmacist extenders, maintaining the security of medications, and protecting the confidentiality of patient information. Provides medication information support for medical and nursing staff, ensuring continuity of care, and identifies potential challenges and intervenes, if necessary, to medication-related issues.

• Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006.
• Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
