Job summary

Hartford HealthCare is seeking a Clinical Pharmacist to optimize drug therapy for patients in a clinical specialty area and to educate pharmacy staff and other caregivers to ensure safe and cost-effective therapy. The position reports to the Pharmacy Manager and involves collaboration with healthcare providers to resolve medication-related issues.

The ideal candidate holds a CT Pharmacist license or eligibility within 45 days, a BS in Pharmacy with PharmD preferred, and ASHP residency or 3–5

Qualifications

• Current CT Pharmacist license or eligible to receive licensure within 45 days of hire.
• Bachelor's in Pharmacy required, PharmD preferred.
• Completion of an ASHP approved pharmacy residency program Or 3 to 5 years pharmacy experience required.
• BCPS is preferred.

Responsibilities

• Evaluates drug literature by analysis of experimental design and methodology to compare therapeutic regimens and ensure cost-effective drug therapy.
• Provides clinical support to staff pharmacists and collaborates with healthcare providers to resolve medication-related issues.
• Designs or develops therapeutic regimens based on patient information to maintain safety of drug therapy.
• Assists in training staff, serves as a preceptor for residents/students, and provides education to nursing staff, physicians and caregivers.
• Participates in Pharmacy clinical support teams and system councils to develop regimens, order sets, and protocols.
