Job summary

Allen County War Memorial Coliseum is seeking a Public Health Nurse for Infectious Disease Case Management to lead education, testing, screening and follow-up care across Allen County, including refugee case management and outbreak investigations.

Under the supervision of the Clinical Services and Case Management Director, you will coordinate TB investigations, administer vaccines, and collaborate with Federally Qualified Health Clinics and other partners.

Qualifications

• Associate’s Degree in Nursing and one year previous nursing experience required. Bachelor’s Degree in Nursing preferred.
• Knowledge of Case Management skills
• Strong written and verbal skills, including public speaking
• Knowledge of accepted principles and practices of Public Health Nursing, with the ability to examine and screen individuals, assess needs, and provide information and referrals

Responsibilities

• Conducts health education, testing, screening and follow-up care programs throughout Allen County.
• Investigates infectious disease cases and performs vaccination and health screenings.
• Coordinates TB investigations, lab coordination, and contact tracing in accordance with laws.
• Coordinates and arranges immunization clinics and mass prophylaxis initiatives.
• Compiles and prepares monthly, annual, and special reports for ISDH and Health Department administration.
