Job summary

Elaya Health in New York is seeking a Nurse Practitioner or Physician Assistant to join our Infectious Disease division providing on-site care to residents of skilled nursing and long-term care facilities. You will evaluate and manage a broad range of infections with strong support from physicians and administration.

The role offers clinical autonomy, flexible scheduling, and opportunities to contribute to a growing specialty program while documenting care in facility EMRs.

Qualifications

• Licensed NP or PA in NY and/or NJ, or eligible for licensure.
• Infectious disease experience preferred but not required.
• Experience in internal medicine, hospital medicine, geriatrics, wound care, or skilled nursing is valued.

Responsibilities

• Perform on-site infectious disease consultations and follow-up visits.
• Evaluate and manage infections in the skilled nursing population.
• Review cultures, laboratory results, imaging, and antimicrobial therapy.
• Manage and monitor patients receiving antibiotic treatment.
• Provide recommendations regarding antibiotic selection, duration, and follow-up.
• Evaluate recurrent or difficult-to-treat infections.
• Collaborate with nursing staff, primary care providers, wound care teams, and facility leadership.
• Help reduce unnecessary hospital transfers through timely specialty evaluation.
• Document in facility EMRs, with training and administrative support provided.
