Job summary

Elaya Health is seeking a Nurse Practitioner or Physician Assistant to join our Infectious Disease division, delivering on-site ID care in skilled nursing and long-term care facilities. You will evaluate and manage a wide range of infections with strong collaboration with facility teams and administration.

This role offers flexible scheduling, strong physician and administrative support, and significant clinical autonomy as the program grows within our expanding specialty division.

Qualifications

• Licensed Nurse Practitioner or Physician Assistant in New York and/or New Jersey (or willingness to obtain licensure).
• Infectious disease experience preferred but not required.
• Experience in internal medicine, hospital medicine, geriatrics, wound care, or skilled nursing is valued.
• Strong clinical judgment and communication skills.
• Ability to work independently while collaborating with physicians and facility teams.

Responsibilities

• Perform on-site infectious disease consultations and follow-up visits.
• Evaluate and manage common and complex infections in the skilled nursing population.
• Review cultures, laboratory results, imaging, and antimicrobial therapy.
• Manage and monitor patients receiving antibiotic treatment.
• Provide recommendations regarding antibiotic selection, duration, and follow-up.
• Evaluate recurrent or difficult-to-treat infections.
• Collaborate with nursing staff, primary care providers, wound care teams, and facility leadership.
• Help reduce unnecessary hospital transfers through timely specialty evaluation and proactive management.
• Document in facility EMRs, with training and administrative support provided.
