Job summary

Cincinnati VA Medical Center is seeking an Advanced Practice Nurse (APN) - Certified Nurse Practitioner (NP) to practice in Infectious Disease. The NP will provide advanced clinical care, consultation, and management focused on quality and timeliness to support the VA mission.

The role emphasizes independent practice, leadership within the Primary Care Team, and education for patients and families in a complex Veterans Affairs setting.

Responsibilities

• Independently manages a panel of patients with acute and chronic health conditions emphasizing health promotion and disease prevention using evidence-based practice.
• Develops and implements individualized plans to achieve optimal population health and disease management.
• Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
• Orders and interprets labs, tests, and diagnostic studies.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals and consultations with specialty services.
• Provides education to patients and their families.
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
