Job summary

Aviata Health Group in Florida is seeking an Infectious Disease Nurse Practitioner to lead clinical care in skilled nursing and long-term care facilities. You will collaborate with physicians, nurses, and facility staff to manage infectious disease programs and drive proactive resident care.

The role emphasizes leadership, comprehensive exams, prescribing treatments, care planning, and discharge coordination, with a focus on reducing re-hospitalizations and improving outcomes for geriatric

Qualifications

Active Florida Nurse Practitioner license (unencumbered).
Master's Degree in Nursing from an accredited program.
Strong clinical background in Infectious disease.
Geriatric or adult medicine experience preferred in skilled nursing/long-term care settings.
Ability to engage with elderly patients and families.

Responsibilities

Perform comprehensive physical examinations and evaluate changes in patient conditions.
Prescribe medications and treatments as needed.
Develop patient care policies and facilitate referrals to specialists.
Coordinate advanced care planning and discharge visits.
Ensure timely communication and documentation of assessments, plans, and follow-ups.
Support physician-patient relationships and address gaps in care.
