Decatur Memorial Hospital is seeking a Nurse Practitioner or Physician Assistant to join its Infectious Disease Team on a full-time day shift. The role focuses on coordinating comprehensive care under physician supervision, including evaluation, counseling, and referrals.

Responsibilities include EMR documentation, patient education, and collaboration with staff and outside agencies. Illinois licensure, required certifications, and a valid CPR/ACLS where applicable are essential.

Qualificações

• APRN or PA licensing required by Illinois law.
• Appropriate APRN/PA certification per department.
• DEA and IL Controlled Substance licenses within 3 months if not held.
• Current CPR certification.
• ACLS where required.
• Minimum 1 year post-graduate experience for mid-level provider preferred.

Responsabilidades

• Provide comprehensive care in coordination with physicians and staff.
• Diagnose, treat, and manage patient care with referrals.
• Document care in electronic medical record.
• Develop patient education and wellness programs.
• Coordinate patient care and family counseling with agencies.
• Perform routine exams and initiate treatments.
