Job summary

Evergreen Center for Health & Rehabilitation is a proud affiliate of National Health Care Associates and seeks an Infection Preventionist to lead infection prevention and control programs in a long-term care environment.

You will develop policies, monitor outbreaks, and educate staff to ensure safety for residents and team members. This role supports a mission-driven, collaborative care culture with opportunities for professional growth.

Qualifications

• Valid state nursing license.
• Degree in nursing, epidemiology, biology or microbiology.
• Infection Control Certification is required prior to assuming the role.
• Experience in a Long-Term Care or similar setting preferred.
• Excellent communication and ability to work in a multidisciplinary team.

Responsibilities

• Oversee infection control program to identify and prevent infections for residents and staff.
• Review and implement infection control policies and procedures.
• Oversee antimicrobial stewardship, employee health, and universal influenza programs.
• Serve as a member of the Quality Assessment & Assurance Committee.
• Ensure compliance with screening, documentation, and immunization reporting requirements.
• Perform rounds to identify hazards and educate staff on infection control.
• Develop and deliver staff education on infection control and surveillance.
• Track and present surveillance trends and coordinate with QAPI actions.
• Maintain records as required by state and federal regulations.
