Job summary

Willow Creek Health Care Center is seeking a knowledgeable, organized Infection Prevention Nurse (LVN) to lead our facility's infection prevention and control program. The role reports to the Director of Nursing and focuses on surveillance, education, and regulatory compliance to protect residents, staff, and visitors.

Qualifications

• Graduate of an accredited vocational nursing program.
• Current California LVN license in good standing.
• Current CPR certification.
• Two or more years of nursing experience; LTC preferred.
• Infection prevention experience preferred.
• Knowledge of epidemiology, surveillance, and isolation precautions.

Responsibilities

• Plan and oversee the facility's infection prevention and control program.
• Monitor infection trends and enforce CDC/OSHA guidelines.
• Educate staff on infection prevention and hand hygiene.
• Collaborate with medical and support staff on care plans for infected residents.
• Assist with outbreak investigations and reporting.
• Maintain infection logs and surveillance data.
• Ensure safe handling of contaminated materials and waste.
• Support employee health surveillance and TB screening.
• Participate in quality improvement and survey readiness.
