Job summary

Trilogy Health Services, LLC in Peru, Indiana, is seeking an Infection Control and Wound Nurse (LPN/RN) to support the DON and ADON by overseeing the wound care program and outlining treatment plans.

You will monitor Braden Scale results and weekly wound measurements, develop and maintain an active Infection Control program, ensure orders and care plans are up to date, participate in surveys, and conduct daily rounds.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Wound Care Certification (WCC) preferred or willingness to complete the certification training program.
• Infection Preventionist certification or willingness to complete the certification training program.
• Current, valid CPR certification required

Responsibilities

• Monitor Braden Scale results, establish preventative measures for high‑risk residents, ensure wound measurements are obtained weekly.
• Develop and maintain an active Infection Control program, ensuring binder and antimicrobial stewardship.
• Ensure orders are in medical records, care plans entered, CNA sheets/profiles updated and other elements including information for hospice and dialysis residents.
• Participate in survey and mock‑survey campus preparedness.
• Make daily rounds of the nursing service department(s) to ensure staff follow standards.
• Review fridge temps, FirstDose room temps BID, crash cart audits, fall intervention, med carts, HIPAA violations, TSS, and med rooms and report findings.
• Provide patient care as needed and participate in campus on‑call rotation.
